Kaiser Permanente —supposed to be different than other Bigs? They are a hybrid hospital/insurer/primary care/etc so supposedly -aligned incentives…with an NPS of 48 vs 0 for UNH 16 for ELV(Anthem) 32 for HUM. HCA is 9 and a negative 17 for CommonSpirit Health it’s “nonprofit” equal. Yet despite $3B in profits , 98% vote to strike in Cali and several other states. Why?#understaffing #caredenials and #skippedbreaks reflect Kaiser is indistinguishable from othe Bigs insensitive to the frontline. The way out-permanently for Permanente:The way out for this and other Bigs (and autoworkers/Hollywood)would be to non magically shave $2-5000 in healthcare costs x 305,000 employees. A billion drops to EBITDA if about $3300 is found using direct care strategies. Non magical. That’s only 14% shaved off their overall cost of healthcare and 60% is possible using all 9 tactics in direct care. Said another way over $4B would drop to the bottom line and cover staffing ratios,breaks and fill essential slots financially AND offering free healthcare for all would be about $1.8B. Doable. Now that’s a union buster. An epic game changer. And because they are their own direct contractor of virtually all 9 tactics(DPC/cash pay surgery/imaging/specialists/labs/nav/claims adjudication/metrics)the 60% savings are closer to 75-80% which is $5B to ebitda. Hey UnitedHealth Group receives 25% of rev-from itself! KP has a good role model. Bigs aren’t bold or imaginative enough to take care of their biggest problem with the only solution -direct care. And the direct movement is antithetical to the volume addiction of most bigs-not Kaiser Permanente. Kaiser Permanente Fined for Limiting Patients’ Access to Mental Health Care At the end of June, the California Department of Managed Health Care (DMHC)announced a $4 million fineagainst California’s largest HMO for limiting patients’ access to mental health care. The fine affirms the findings of an exhaustive complaint filed by Kaiser Permanente’s frontline mental health clinicians, who are represented by the National Union of Healthcare Workers (NUHW). The short-staffed agency has been inundated with COVID-related complaints. Between February and September of last year, it conductedon-site inspections for only 5%of almost 7,000 pandemic complaints.Almost half of Cal/OSHA’s citations to hospitals are directed at Kaiser. But it’s difficult to know for sure why Kaiser has been disciplined the most, said Laura Stock, director of the Labor Occupational Health Program at UC Berkeley. In addition to the $4 million fine, the DMHC also filed acease and desist order”against Kaiser that emphasizes the risks posed to patients:“The Department finds that the Plan’s deficiencies are serious and may put some of its members at risk of harm. Therefore, as set forth in this Order, the Department of Managed Health Care hereby directs the Plan to Cease and Desist from any further violation of the foregoing statutes and regulations in order to protect the interests of enrollees.” (p. 7, Cease and Desist Order)In apress release,DMHC Director Brent Barnhart reiterated the seriousness of the DMHC’s findings:“The Department’s actions are a result of both the seriousness of the deficiencies and the failure of Kaiser to promptly correct them. Are healthcare workers just using patients as a front so that they can do less work, or are there real concerns with patient care.We explore the relationship between RN concerns and care quality in a white paper that we just released. https://lnkd.in/gFtbA9JX This research is based on nearly 1,000 RN interviews in the past 3 months, and correlations to quality metrics. Bottom line--we need to take care giver concerns about quality very seriously. Their feedback lines up tightly with other care metrics. If the people working at the bedside say that staffing shortages are hurting, even killing patients, we need to listen and work together to find solutions. There are no easy solutions for hospitals--this will take everyone working together!

I've said it before and I will say it again. I am not a nurse. I am not a hospital executive. I am, however, an advocate for change. "The strains of an acute staffing shortage have led union officials to warn that the strike is likely if Kaiser executives ignore their concerns. Workers say the lack of adequate staffing at Kaiser facilities is creating unsafe conditions for patients. The unions say that Kaiser needs better wages to attract workers and that it needs to hire enough people to make up for the exodus of staff during the pandemic." -New York Times (Kaiser Permanente Poised to Strike, 10/3/2023)This feels different. In the healthcare staffing world, Kaiser is known to have the some of best overall salaried positions for nurses and other healthcare workers, along with great healthcare benefits and reasonable patient case loads. On this day, more than 75,000 healthcare workers have threatened to walk off, placing Kaiser Permanente and more importantly, the patients in an impossible situation. Should the strike begin, it would be "the largest in US History."Post-COVID: Like many hospitals systems throughout the country, Kaiser is still struggling with maintaining consistent staffing numbers, as they continue to try and promote a healthy work environment. So, what is this really about? Overall Value? Do healthcare workers come to work feeling valued and seen?In an article published by the Harvard Business Review in March 2023, Patrick T. RyanandThomas H. Lee, stated "Competitive pay and other support options are essential torecruitingcaregivers, of course, but organizational culture, including a commitment to excellence, is what makes themstay." A commitment to excellence is also a commitment to staff, but most important, it's a commitment to the patients. That commitment to excellence doesn't have to advertise higher pay. It doesn't have to rely on outside parties to determine competitive wages. ✔ A commitment to excellence listens. ✅ A commitment to excellence is humble.☑ A commitment to excellence is fair. It makes me wonder though: if healthcare workers within a well known, well established organization like Kaiser are feeling this way, what do the others around the country feel?
